A new patient will not be a good candidate at this time if: under 18 or over 70 years old, pregnant, history of intracranial pressure, actively struggling with illicit substance abuse, serious heart issues, uncontrolled high blood pressure, taking/using kratom, psychosis, active methadone, suboxone or clozaril/clozapine prescription.

Choose can provide you with a Superbill which you can provide to your insurance provider. A Superbill is an itemized receipt of the services provided to the patient. It is your proof of service"
